Community Services The establishment of Commonwealth Carelink Centres seeks to enhance the quality of care delivered to older Australians and younger people with disabilities, through the expansion of relations between general practitioners, community care service providers, and the wider community. Health Providers The establishment of Commonwealth Carelink Centres will significantly contribute to the success of the key components of the Government’s EPC package: the extension of the coordinated care trials and the inclusion of the general practitioners General Practitioners The Commonwealth Carelink Program was developed as a component of the wider Enhanced Primary Care (ECP) package, and will support the introduction of other EPC incentives that encourage general practitioners to participate in care planning, case conferencing and voluntary health assessment for individuals aged 75 years and over, and for indigenous people aged 55 years and over. Service Providers Commonwealth Carelink Centres offer a new way for all Australians to access information about community and other aged care services, with a single phone call or visit to a Commonwealth Carelink Centre shopfront. The Centres act as a single point of contact, providing reliable information and guidance about community care services and aged care homes available in the local community.
